As a program licensed by the State of Connecticut Office of Early Childhood, Summer Place is required to have a Physician Medical Record on file for all campers. Information on the form must be based on a physical exam conducted after August 8, 2022. Please be advised that any child not in compliance with this regulation will not be permitted to attend camp. Medical records are not kept on file from year to year; you must provide a new medical form each year. Your child may not attend camp until the properly completed forms have been received. Medical forms are due by May 1, 2025.
Parents must complete and fill out page 1 of our camper medical form. Your child’s physician must complete page 2. Completed forms should be emailed to splace@hartford.edu. Forms cannot be uploaded to the registration site.
If your child needs to take medication (prescription or non-prescription) while at camp, or have it on hand, you and your child’s physician must complete an Authorization for the Administration of Medications Form, as well as an Individualized Plan of Care Form. It is imperative that the nurse and our staff be aware of any special medical needs your child may have. This form is found on our website. Please let us know if you would like to speak to the nurse before the opening of camp. Please note: Medication must be in original container with label.
Individualized Plans of Care: The State of Connecticut requires all camps to fill out an Individual Plan of Care for any child who has a health-care need or disability that requires special consideration while at Summer Place. The reason for the Individual Plan is to make sure the Summer Place staff are aware of the camper’s situation and what to do in case of an emergency. Individual Plans of Care will be sent out by email and need to be signed and returned before a camper can attend Summer Place.
