Vaccine Survey for Employees
As more people become vaccinated, state and federal COVID-19 public health guidelines continue to change. Many of these guidelines, including quarantine requirements, hinge on whether an individual has been fully vaccinated against COVID-19. As such, a person’s vaccination status will be key to understanding and complying with current and future public health guidelines as we move through the summer and towards the fall.
Most public health experts agree that having at least 70 percent of the population vaccinated is key to combating the virus and ending the pandemic. Information from employees will help us determine if we’ve achieved that target number and will allow us to determine what health and safety protocols will be in place this fall.
We have not yet mandated the vaccine for employees; however, this may change and we strongly encourage each member of our community to be vaccinated. To help in this effort, we encourage you to respond to this very brief survey with your current vaccination status. This survey does not ask for any identifying information, and will only be used to help us determine our community vaccination rate and to make informed decisions on policies and guidelines. The data will be securely stored by the Office of Institutional Effectiveness and will only be examined and shared in aggregate.
If you have been vaccinated, you will be invited to submit your proof of vaccination on the University’s secure Health Services portal, PyraMed. While this is not mandatory at this time, it is strongly encouraged. More detailed information about PyraMed and the storage of records is included in the survey.
