Course Override Request Form

This form is used to request permission for an override into a UIS category course (ex. UISA, UISC, UIST and UISS). Each form submission is for one course request. If you are requesting more than one course override, you will need to fill out the form again. The decision to provide an override into a course is at the UIS department’s discretion.
