Special Events Intake Form
- Download the Special Event Toolkit and Operations Plan form.
- Open the document and enable macros and content control, if prompted.
- Click the hyperlinks on the form to assist you in knowing policies and procedures regarding Special Events management within the Student Centers and associated facilities.
- Fill out the form based on the information that you have regarding your Special Event.
- Once completed, you may submit the form by:
- Submit the form directly using Microsoft Outlook and Microsoft Word 10 and up, by clicking the "Submit" button at the end of the form. Or...
- Attach a completed copy to an email and send to firstname.lastname@example.org. In the subject line, please put "Operations Plan Submission.
